 ABOUT OUR COMPANY

Citizens Choice Physiocare, Inc. is formerly known as Citizens Choice Home Care Services, Inc. (CCHCS).  CCHCS, a licensed provider of home and community-based care, out-patient behavioral health services, and non-emergency medical transportation (NEMT), was founded in 1987 initially as a home health agency. In 2002, CCHCS contracted as a provider of non-emergency medical transportation due to a need for services throughout the city.  Sometime later CCHCS contracted with the Aging Commission of the Mid-South.  In addition, CCHCS extended its contracting partnership with three Managed Care Organizations (Blue Cross Blue Shield, United Health, and Amerigroup) under the CHOICES program, providing personal care, attendant care, in-home respite, and out-patient behavioral health services.


CCHCS transitioned to the physical therapy industry as Citizens Choice Physiocare, Inc., a physical therapy practice, offering premier rehabilitative services to improve overall well-being and fitness, and sports performance.  Although our service industry changed, the quality of care remains.  Throughout the years we have been a valued provider to populations citywide, providing the highest degree of care in the services offered, combined with compassion for our clients’ wellness and well-being.

Our purpose is to ensure and promote continuous improvement in our clients’ daily lifestyle.  A system of care is implemented according to the needs of each client, with goals to recovery. Our clients’ needs are central to effective practices and plans of care. Citizens Choice have serviced residents throughout Memphis/Shelby county for many years.  We continue striving to be one of the industry's leading providers, while advancing hope, resiliency, recovery, and full continuity of care leading to positive outcomes. Our vision is to focus on orthopedics (sports injuries, workplace injuries, etc), pediatrics, and geriatric physical therapy. Citizens Choice offers manual physical therapy as needed, in addition to physical therapy utilizing devices and machines, PT modalities, stretching and strengthening. Our practice have the capability to help individuals return to their prior level of functioning and lifestyle, and improve overall health and well being by providing effective treatments via in-person and/or telehealth with the power of digital tools.

Orthopedic physical therapy can have a tremendous impact on increasing mobility, relieving pain, and even correcting damage to the skeleton.  A skilled physical therapist (PT) can get you back on track with your daily activities after surgery, an injury, accident, or illness.  It is very common to find patients with trauma to the bones, muscles, tendons, joints, and ligaments. Orthopedic physical therapy involves the care of your entire musculoskeletal system, which includes your:

Orthopedic physical therapy works to integrate all your bodily systems to treat your injury or condition appropriately.

This type of physical therapy treats children for a variety of different reasons, including bone/muscle issues, sports-related injuries, or genetic, brain, spine, or nerve disorders.  It can also help correct birth defects, delayed physical growth, and treat acute damage.  Therapy strengthens the affected parts and perfect the movements, which can make children comfortable and engage them in playing times.  


Pediatric physical therapy sessions will look and feel like play. The PT will help improve gross motor skills (tasks that involve large muscle groups, like walking and throwing) by encouraging kids to do things like:

  • Play on large exercise balls to build strength

  • Run/hop around to improve their coordination

  • Balance on a balance beam

  • Stand on one foot

As our bodies age, we lose strength, muscle mass, bone density and balance reactions. Physical therapy can play an important role in lowering your risk for falling as you age, and in preserving your independence. Physical therapists are skilled in working with people affected by these challenges.

One unfortunate circumstance that does come with participating in sports is the possibility of injury.  And physical therapy has been a continuing mechanism for athletes looking to recover from injuries and/or improve their performance.  Some physically demanding sports may require immediate pain relief for the athletes. Telehealth (Virtual Physical Therapy)

COVID-19 changed not only the world, but how healthcare is being delivered, as “social distancing” became the norm.


How is telehealth physical therapy delivered?


Through virtual or live video, telehealth PT is referred to as “synchronous” format, which uses live interaction between two parties over video in “real-time.” 


Some common uses for telehealth PT:


    •    Prevention of readmission through home safety evals and mobility screens

    •    Post-discharge checkups and safety screens

    •    Advanced intake of subjective history

    •    Wellness and preventative services to avoid hospitalizations

    •    Consulting with other practitioners (think McKenzie screens, CrossFit/yoga instructors, or pelvic floor consults)

    •    Post-surgical monitoring and rehabilitation

    •    Q and A to prevent hospital readmissions and future injuries


Standards of practice still hold for telehealth PT, including:


    •    Informed consent

    •    Abiding by HIPAA

    •    Protecting patient privacy

The consultation is crucial to your safety and care.  Direct access allows you to begin physical therapy and related services without a physician’s referral in most states where direct access is available.


During the consultation you and the therapist will go over any concerns or questions you may have regarding your treatment. The complimentary consultation will also allow the therapist to evaluate your condition with your medical history, lifestyle questions, level of pain, and areas of affliction. This will help the therapist get familiar with your specific situation and understand how your condition is affecting your life, and recommend an individualized treatment plan. 


Physical therapy can help you regain a pain free life after an injury, and return to your previous level of functioning. We are committed to results and helping you achieve full mobility and rest easy again. 


Questions you might have during the consultation:

  • What type of physical therapy will be provided for my condition?

During your first appointment, the therapist will ask you about your particular issue.  The options include manual therapy, therapeutic exercises, balance training, pain modalities, massages, etc.

  • Will my insurance cover the treatment?

If your health plan covers the treatment, you may only need to pay the copay and coinsurance. Treatment may be covered completely, partially, or not at all.

  • How many physical therapy sessions will I need?

The frequency of your sessions will depend on the severity of your diagnosis, your personalized plan based on your lifestyle and schedule, and your progress during each session. The therapist will go more in-depth on frequency, and recommendations for a successful recovery.

  • How long are the sessions?

The therapist should be able to let you know the duration of each session. 

The sessions can vary from 30 minutes to over an hour.

  • Who will I see during the sessions?

We will make every effort to set your appointments with the same therapist, remember that this might not always be possible.  It's also possible that you will do some one-on-one sessions with your primary therapist, but will have a licensed physical therapy aide (PTA) assist you with some of the exercises.

  • How soon can I expect to return to my normal routine or sports?

The therapist can show you exercises, stretches, and mobility movements to practice at home between your sessions. Instructions will also be provided for you to follow. The independent management of your condition will further promote recovery sooner.

  • Can I expect to fully recovery?

While many patients are able to regain their body’s normal function in only a few sessions, some may take longer (some take years) to achieve their desired outcome. This means that follow-up sessions may be necessary, which can be frustrating for the patient whose only hope is to be well.

Protecting Patients and Employees
