In 2020, a global pandemic changed the very fabric of societies near and far, prompting innovation and a rise in new technologies and treatment modalities in the healthcare industry. As such, healthcare practitioners made the necessary strides to ensure health services remained accessible. Online video and audio appointments began to surge. Virtual care is "not only possible, but preferable." Modern Telehealth has taken virtual care "beyond the lack-buster offerings of the pre-COVID era." Citizens Choice Physiocare, Inc. (CCPC) was rebranded as a physical therapy practice/NEMT provider and is here to empower you to take control of your physical well-being, bringing Virtual Physical Therapy via Telehealth, and Non-Emergency Medical Transportation services (NEMT). Headquartered in Memphis, Tennessee, CCPC formerly known as Citizens Choice Home Care Services, Inc. (CCHCS), a licensed home and community-based services (HCBS) agency that provided personal support, attendant care, in-home respite, outpatient behavioral health, and NEMT services was founded in 1987. When a growing need emerged, CCHCS launched its NEMT services in 2001. In May 2021, the company made its transition from HCBS to specialty services as a physical therapy practice (PT), while preparing to resume NEMT services.
CCPC's premier virtual integrated rehabilitation services are available via one-on-one, face-to-face outpatient PT eVisits (in approved locations, if applicable), and virtual check-ins. We utilize patient-friendly and HIPAA compliant technology similarly to the growing trend of other practice models to "scale the human connection, to help health plans, healthcare systems, and hospitals extend their reach and achieve their value-based service," and to improve our clients' overall well-being, fitness, and sports performance.
At CCPC, we make accessing PT and NEMT services easier. Virtual PT (Telehealth) services are available throughout Tennessee enabling our PTs to expand their reach. In addition, a growing demand signified CCPC's readiness to expand its Virtual PT operations. Expansion plans include the following jurisdictions: Arizona, Georgia, Iowa, Kentucky, Maryland, West Virginia, and North Carolina. NEMT services are now available in the following Tennessee Counties: Shelby, Madison (Jackson, TN) and Davidson (Nashville, TN), with expansion plans into Georgia, and the District of Columbia. Our Purpose and Vision
CCPC's purpose is to ensure and promote continuous improvement in our clients’ daily lifestyle, utilizing an ideal platform for remote delivery of physical therapy. A system of care is implemented according to the needs of each client, with goals to recovery. While client needs are central to an effective standard of practice and plan of care, our home-based virtual PT is expected to be provided at the same level of care as clinic-based therapy. In addition, clients who may benefit more from hands-on PT are appropriately referred to their medical physician and/or an in-clinic physical therapy provider. MAKING A BIG DIFFERENCE IN VIRTUAL PT FOR CHILDREN AND YOUTH
This type of physical therapy treats children for a variety of different reasons, including bone/muscle issues, sports-related injuries, or genetic, brain, spine, or nerve disorders. It can also help correct birth defects, delayed physical growth, and treat acute damage. Therapy strengthens the affected parts and perfect the movements, which can make children comfortable and engage them in playing times.
Virtual pediatric rehab is a "delivery method for therapeutic intervention, and it's one that succeeds in ways that traditional therapy applications cannot" (Christensen, 2022). Sessions will look and feel like play. THE BENEFITS OF VIRTUAL PT: IS IT EFFECTIVE?
Telehealth, or virtual physical therapy, offers an alternative option for you to receive specialized care from a provider who may be an expert in the care you need despite living thousands of miles apart. (2) This option takes advantage of technology that’s available right at your fingertips. It refers to physical therapy sessions that are held through videoconference calls with a [licensed] therapist through a mobile device. (1) Telehealth may also be referred to as “synchronous” format, using live interaction between two parties over video in “real-time.”
Questions have been raised about the effectiveness of virtual physical therapy. Research has found that telehealth is equally as effective compared to traditional PT. The same exercises that your therapist shows you through your computer or phone screen are the same exercises that they will walk you through when they come to your home. (1) Even without hands-on techniques, physical therapists are able to provide the same guidance for therapeutic exercises to patients through a computer as they can in person. Virtual physical therapy also helps patients learn how to do the exercises and techniques on their own. That way they can continue performing them between sessions. (1) Further, studies have shown that treatment outcomes for virtual physical therapy appear to be on par with the outcomes for face-to-face physical therapy sessions. (2) A study published in The Journal of Bone and Joint Surgery showed that tele-rehabilitation was similarly effective as traditional care for patients recovering from total knee arthroplasty.(4) The Musculoskeletal Journal of Hospital for Special Surgery discovered that, during COVID-19, patient satisfaction with virtual physical therapy was comparable to in-person services. (5) The American Physical Therapy Association (APTA) advocates for telehealth as an effective and viable method for delivering patient care.(6) Some common uses for Telehealth PT:
• Prevention of readmission through home safety evals and mobility screens
• Post-discharge checkups and safety screens
• Advanced intake of subjective history
• Wellness and preventative services to avoid hospitalizations
• Consulting with other practitioners (think McKenzie screens, CrossFit/yoga instructors, or pelvic floor consults)
• Post-surgical monitoring and rehabilitation
• Q and A to prevent hospital readmissions and future injuries
Standards of practice still hold for Telehealth PT, including:
• Informed consent
• Abiding by HIPAA
• Protecting patient privacy

INSURANCE/PRIVATE PAY COSTS/BILLING AND ONLINE PAYMENT OPTIONS
CCPC's goal is to make virtual PT services financially accessible to as many as possible. Many insurances are now covering Telehealth in adherence to government mandates.
How Do I Pay for Telehealth?
Talk with your insurer to find out if telehealth for physical therapy is covered. If you're uninsured or have exhausted benefits, you have the option to pay direct for telehealth PT. We will submit claims to most major insurance carriers.

No Charge Consultation (15 minute)
Direct access and the consultation allows you to begin physical therapy and related services without a physician’s referral in most states where direct access is available.
Initial Assessment
During the initial assessment you and the therapist will go over any concerns or questions you may have regarding your treatment.
The initial assessment will also allow the therapist to evaluate your condition with your medical history, lifestyle questions, level of pain, and areas of affliction. This will help the therapist get familiar with your specific situation and understand how your condition is affecting your life, and recommend an individualized treatment plan of care (POC).
Questions you might have during the consultation:
1. What type of physical therapy will be provided for my condition?

During your first appointment, the therapist will ask you about your particular issue. The options include therapeutic exercises, balance training, pain modalities, etc.
2. Will my insurance cover the treatment?
-
If your health plan covers the treatment, you may only need to pay the copay and coinsurance. Treatment may be covered completely, partially, or not at all.
3. How many physical therapy sessions will I need?
-
The frequency of your sessions will depend on the severity of your diagnosis, your personalized plan based on your lifestyle and schedule, and your progress during each session. The therapist will go more in-depth on frequency, and recommendations for a successful recovery.
4. How long are the sessions?
-
The therapist should be able to let you know the duration of each session. The sessions can vary from 30 minutes to one hour.
5. How soon can I expect to return to my normal routine or sports?
-
The therapist can show you exercises, stretches, and mobility movements to practice at home between your sessions. Instructions will also be provided for you to follow. The independent management of your condition will further promote recovery sooner.
6. Can I expect to fully recovery?
-
While many patients are able to regain their body’s normal function in only a few sessions, some may take longer (some take years) to achieve their desired outcome. This means that follow-up sessions may be necessary, which can be frustrating for the patient whose only hope is to be well.
